Long-Term Care & Gerontology Program
Our Long-Term Care & Gerontology Program is focused on the unique needs of seniors and long-term care clients, their families and their caregivers. Recognizing the ever-changing needs of our clients, our approach is client-driven, respecting their right to choose, and fostering well-informed decision-making on their part. Our experienced and knowledgeable multidisciplinary care providers:
- Educate clients, families and caregivers about all aspects of their conditions, realistic outcomes and alternatives to care
- Provide coordinated, holistic care that encompasses the individual’s physical, social, cultural, psychological and spiritual needs
- Recognize the individual’s abilities and disabilities, degree of illness or wellness and available support systems
- Provide continuity of care and communicate with other members of the care delivery team
- Recognize and assist in the identification and resolution of at-risk or unsafe situations
- Recognize and respond to unmet needs, through collaboration, advocacy and referral to other community organizations
- Draw on the advanced knowledge and experience of Saint Elizabeth Health Care’s wealth of clinical experts
At the organizational level, Saint Elizabeth Health Care strives to continually enhance our Long-Term Care & Gerontology Program by:
- Anticipating and responding to the changing needs of clients in creative ways
- Developing services that are based on best practices and innovative service delivery models
- Promoting partnerships that bridge gaps and enhance and support services in the areas of senior care and long term care
- Supporting and promoting research opportunities
- Continually evaluating service delivery, educational and research activities
- Supporting ongoing staff education about the latest developments and approaches to care
