Saint Elizabeth Health Care’s Rehabilitation Program consists of four services:

  • Occupational Therapy
  • Driver Assessment and Training
  • Physiotherapy
  • Speech and Language Pathology

All of our health professionals providing these services to clients whose lives are affected by physical illness or injury, congenital or developmental disorders, mental illness, the aging process, palliative care and/or psychosocial problems.

The goal of the rehabilitation program is to treat illness and injuries through high-quality, cost-effective therapy services which are individually tailored to maximize independence and recovery, delivered in partnership with the person’s circle of care, to affect a seamless continuum of care.

Integral to the rehabilitation process is the completion of a comprehensive assessment/consultation, goal-directed and client-focused treatment plan, advocacy, and on-going client and family education with respect to prevention and health promotion. Recognizing that people recovering from illness or accidents require ongoing rehabilitation and reintegration in their home, school, work and community environments, our experienced multidisciplinary team work collaboratively with appropriate community resources to provide clients and their families with a comprehensive range of services. For example, we integrate access to funding from public and private sources to share cost of care, equipment needs and improve home and vehicle access. Where required, we also make appropriate referrals to other community programs.

Services are available upon discharge from hospital, while the client is at home and has made either a self-referral or through their family physician, and when the client is transferred to a long-term care facility. We also offer services while the client is in hospital in order to provide a safe and seamless discharge to their home environment.
