The Smooth Transitions program is designed for patients 65 years of age and older who no longer require acute care treatment and are being discharged from hospital to home.

Smooth Transitions offers:

• Assistance with arranging transportation
• Accompanying the client home
• Environmental risk and safety check
• Picking up/dropping off prescriptions
• Picking up assistive devices
• Light grocery shopping and meal preparation
• Light laundry and linen change
• Light housekeeping
• Follow up call or visit
• Assistance and linkage to other community support services as needed
