Service Coordinator

POSITION SUMMARY:

 

Responsible for the efficient and effective scheduling of clients with a focus on building and maintaining positive internal relationships with front line staff and other administrative staff.   All activities are conducted within the policies, procedures and standards of confidentiality of Saint Elizabeth.

 

POSITION RESPONSIBILITIES:

 

  • Respond to all inbound calls from front line staff and take immediate action as needed
  • Assign primary and/or partner care provider to all unbooked visits to ensure continuity of care in the provision of service delivery
  • Adjust schedule plans in accordance with changes in the service plan in an appropriate and cost-effective manner and in consultation with the front line staff and management and/or their delegate if necessary
  • Adjust schedule plans in accordance with changes in staff availability (book offs, sick days) in Procura in an appropriate and cost-effective manner and inform front line staff of new assignments for same day
  • Maintain availability in Procura as outlined in the master schedule
  • Maintain automated scheduling systems that reflects current client/employee preferences
  • Investigate Not Seen Not Found (NSNF) visits as forwarded by the CellTrak Coordinator
  • Provide follow-up on missed visits as forwarded by the CellTrak Coordinator
  • Discharge clients in Procura when service ends
  • Maintain continuous information flow amongst internal staff (i.e., medical orders, referrals, client information)
  • Maintain data quality and integrity of client information in Procura
  • Provide excellent customer service to internal and external customers with efficiency, courtesy and appropriate urgency

 

QUALIFICATIONS:

 

  • Bilingualism is strongly preferred
  • Relevant post-secondary education (Human/Social Services background preferred)
  • MS Office computer skills at an intermediate level
  • Experience with management of multiple databases
  • Courses in medical administration or health care training are preferred

 

POSITION REQUIREMENTS:

 

  • Familiarity with community health care services and client service coordination experience is preferred
  • Adaptability and an ability to deal with uncertainty
  • Demonstrated creative problem solving
  • Ability to prioritize and deal with tight deadlines
  • Intermediate reading comprehension and excellent verbal communication skills
  • Efficient and accurate data entry skills
  • Exceptional organization, planning and time management skills
  • Ability to work independently and as part of a team
  • Experience in analytical analysis and critical thinking
  • Flexible to work between 7am-8pm
  • Proficiency in Procura and ACS is strongly preferred
